Develop and implement a system-wide plan for at-risk transitional age youth focused on improvements in five areas: education and training, housing, employment, health and well-being, and social connection.
Provide outreach and public benefits enrollment/employment assistance for individuals who are experiencing or at-risk of homelessness, those recently housed, and individuals with significant behavioral health needs.
