The REACH Healthcare Foundation today released a Building Connections to Coverage and Care Request for Proposal (RFP) to support nonprofit organizations engaged in health care connections, health care navigation and enrollment assistance. The RFP is part of the foundation’s Bridge the Coverage Divide investment priority, and specifically the strategy to connect health care consumers to culturally appropriate providers, health insurance, and other public benefits.
The Building Connections RFP is open to nonprofit organizations, governmental or quasi-governmental entities that currently provide outreach and enrollment assistance and health care connector services or are interested in expanding current client assistance to include these services:
- Coverage enrollment: Enrollment outreach and assistance with Healthcare.gov, the Children’s Health Insurance Program (CHIP), KanCare in Kansas and MO HealthNet in Missouri.
- Public Assistance enrollment: Enrollment outreach and assistance for persons and families living in poverty to enroll in publicly funded programs including, but not limited to Temporary Assistance for Needy Families (TANF), Supplemental Nutrition Assistance Program (SNAP), the Child Care Assistance Program (CCAP), Supplemental Security Income (SSI), and Women, Infants, and Children (WIC).
- Care connection: Support to clients in locating and connecting to accessible, affordable, quality, health care services.
Grant awards will be up to $45,000 for a 12-month period, beginning July 2022. The deadline to apply is May 13, 2022. Award notifications will be announced on June 15, 2022. [The deadline to apply is now closed.]
