Support to increase access to mental health services for students at Wyandotte High School through the use of an Engagement Specialist.
Support care coordination for refugees and immigrants.
Provide health and social service supports for 150 refugee individuals and families.
Provide outreach and public benefits enrollment/employment assistance for individuals who are experiencing or at-risk of homelessness, those recently housed, and individuals with significant behavioral health needs.
Provide consumers with health insurance marketplace information and enrollment assistance for coverage and other public benefits.
