The REACH Foundation has invested in several, large-scale projects that engaged other health foundations and extended over multiple years. These projects represent some of our signature investments that have strengthened health care systems and the health care safety net in our service area.
The REACH Foundation introduced a Cultural Competency Initiative in 2008, now the Cultural Competency Collective of Greater Kansas City (www.culturalcollectivekc.org) with an aim of increasing understanding and practice of cultural competency in health and human service organizations — and with a long-term goal of reducing barriers that contribute to health disparities. The REACH Foundation began exploration of the patient-centered medical home model of care in 2007, gathering safety net health care and other health providers to discuss the model and its benefit to health consumers in the foundation’s service area. The foundation’s Medical Home Initiative provided intensive technical assistance and coaching to safety net health care clinics to help them implement recognized organizational and patient care practices defined by the National Committee for Quality Assurance (NCQA) as medical home standards. Over a 10-year period, more than seven Federally Qualified Health Centers and community health centers attained NCQA recognition as patient-centered medical homes.
