  • This Privacy Policy explains how REACH Healthcare Foundation (‘REACH Healthcare”, “we” or “us”) uses, protects or otherwise handles your personal information when you interact with our website at https://reachhealth.org/ (“Site”), we access information through our social media platforms such as Facebook, LinkedIn or Twitter, and our related applications and services on our Site (collectively the “System”). 

We may update this Privacy Policy from time to time and will mark new versions with a corresponding “Last Updated” date. We recommend you visit the Site to review any changes in our practices. Your continued use of the System constitutes your agreement to our then-current Privacy Policy. 

What personal information do we collect from you?  

We collect information from you when you visit or interact with our System, subscribe to a newsletter, fill out a “Contact Us” form, register for an account or login as a grantee, grant seeker or board member, view our blog posts or content, tag us and share our content on social media, or as part of other online services. 

The information we may collect includes, but may not be limited to: 

  • Name, email address, phone number, mailing address, and other location or contact information 
  • Your company name, role, and business affiliations 
  • Internet or other similar network activity, such as IP address, browsing history or information on a user’s interaction with a website or application 
  • Professional or employment-related information, such as when you reach out to us on behalf of your organization or projects to become a grantee 
  • Education Information, such as education history or level of education 
  • Other information related to your grant applications, and requests or inquiries or information you input into the “Message” box to contact us  

Information Collected Automatically

To provide you with services, complete your requests and maintain a functional System, we may automatically collect certain information when you visit or navigate the System, including:

  • Location, date, and time you access the System, and IP addresses to measure our website traffic and to help provide a more personalized experience;
  • Your browser type and other technical specifications such as device ID, hardware/software type, internet service provider, serving domains;
  • Your search history relating to the use of our System;
  • Files that allow us to “recognize” you when you navigate the System, to collect data such as analytics, page views, targeting data, ad views, and interaction data; and
  • Your activities on the System during a session.

Service Providers 

We may disclose your personal information with our contracted service providers who need access to such data to carry out work on our behalf. Business Transfers 

If we are going to be, or are, acquired by or merged with another company, if substantially all of our assets or stock are going to be or are transferred to another company, or as part of a bankruptcy proceeding, we may transfer the personal information we have collected from you to the other company or as part of that proceeding. 

Compelled Disclosure 

REACH Healthcare reserves the right to use or disclose information, including personal information, if required by law or if REACH Healthcare reasonably believes that use or disclosure is necessary to protect REACH Healthcare’s rights and/or to comply with a judicial proceeding, court order, subpoena, or legal process. In certain situations, REACH Healthcare may be required to disclose your personal information in response to lawful requests by public authorities, including to meet or assist national security or law enforcement requirements. 

We may also disclose your personal information as required by law, such as to comply with a subpoena or other legal process, when we believe in good faith that disclosure is necessary to protect our rights, protect your safety or the safety of others, investigate fraud, or respond to a government request.

Through our accounts on social media platforms, we may in some instances collect personal information when you interact with the REACH Healthcare accounts or otherwise communicate with us through those accounts. Any information that you post on social media is governed by each social platform’s privacy notices, and any personal information that we collect via our social media accounts will be processed in accordance with this Privacy Policy.

What ‘cookies’ and similar tracking technologies do we use? 

Cookies are small data files that help us keep track of your usage of the System. Many of the cookies we use enable the System to function as you would expect it to. Persistent cookies enable the System to remember things like your login information for convenience, and session cookies are useful for things like keeping the items in your shopping cart when you navigate away from the page, all of which allows us to enhance the user experience. We use the following four categories of cookies on the System (Strictly Necessary, Functionality, Analytical/Performance Cookies, and Targeting): 

Essential Cookies. These cookies are necessary for the website to function and cannot be switched off in our systems. They are usually only set in response to actions made by you such as setting your privacy preferences, logging in, or filling in forms. 

Functional Cookies. These cookies enable the website to provide enhanced functionality and personalization. They may be set by us or by third party providers whose services we have added to our pages. Still, certain functions or features may become unavailable if you refuse them. 

Analytics Cookies. These cookies allow us to count visits, which pages you visit, and whether you experience any errors and traffic sources so we can measure and improve the performance of our System. They help us to know which pages are the most and least popular and see how visitors move around the System. For example, we use HubSpot as one of our online analytics providers. The HubSpot tracking code monitors website traffic by collecting personal data such as IP address, the company domain address of corporate visitors to our System, timestamps, and pages viewed. HubSpot may use this information to improve its products and services. These cookies do not collect any personally identifiable information. The information collected is only used to analyze how the System is being accessed and used in order for us to improve the System and your experience using it. 

Targeting Cookies. These cookies may be set through our System by our advertising partners. They may be used by those companies to build a profile of your interests, collect information designed to customize your experience on the System and other sites, show you relevant advertisements. and tailor marketing to your interests. This information may be shared with third parties for marketing purposes.

Opting out

If you do this, however, you may have to manually adjust some preferences every time you visit a website, including ours, and some services and functionalities may not work. Google 

 We, along with third-party vendors such as Google use first-party cookies (such as the Google Analytics cookies) and third-party cookies (such as the DoubleClick cookie) or other third-party identifiers together to compile data regarding user interactions with ad impressions and other ad service functions as they relate to our website. Users can set preferences for how Google advertises to you using the Google Ad Settings page. Alternatively, you can opt out by visiting the Network Advertising Initiative Opt Out page or by using the Google Analytics Opt Out Browser add on 

Marketing Communications 

You have the right to opt-out of marketing communications we send you.  

If at any time you would like to unsubscribe from receiving future emails, follow the instructions at the bottom of each email and we will promptly remove you from ALL correspondence. Please be aware that you will continue to receive certain communications from us that are necessary for our services.  

Your Choices

  • access and data portability of the Personal Information we have collected, used, or otherwise processed and you can confirm what Personal Information has been collected.  
  • correct inaccuracies in the Personal Information we may have   
  • delete your Personal Information

Do Not Track Signals 

The System does not respond to browsers’ Do Not Track signals.  

Children Under the Age of 16 

Our System is not designed or targeted for children, and we do not knowingly collect, maintain, or use personal information from any person under the age of 16. If you learn that a child under the age of 16 has provided us with Personal Information in violation of this Privacy Policy, then you may alert us at info@reachhealth.org. We will take reasonable steps to delete that information as quickly as possible.
