This year, the REACH Foundation awarded grants totaling $605,000 to 14 organizations to support the use of care coordinators, community health workers, and other connectors to help uninsured and medically underserved individuals access health services.
The 12-month funding initiatives are intended to provide connector and wrap-around services for residents to various essential resources, including healthcare, mental health support, dental care, and various community assistance programs. These services aim to create a comprehensive network that addresses the community’s diverse needs, ensuring that individuals receive the holistic support necessary for their overall well-being.

The services these dedicated organizations provide are essential in bridging the gaps in coverage and care for everyone,” said Vice President of Programs Carla Gibson. “These organizations exemplify our commitment to meeting the evolving needs of our targeted populations, tirelessly working to create a healthier, more inclusive community where every individual can thrive.”
Community health workers, case managers, and promotoras are crucial in care connectivity. The primary purpose of care coordinators is to bridge gaps in language, literacy, and cultural understanding and navigate personal life experiences. This enables individuals to effectively learn how to maneuver through complex healthcare systems and access vital services. By fostering these connections, they empower underserved populations to overcome barriers and secure the healthcare they need.
