Program grants fund health care services, public policy and advocacy activities, and service integration and expansion efforts.
Program grants may be used for salary expenses of health care staff and other key program personnel, equipment, supplies and other direct program expenses. These grants generally will not cover non-health related expenses, purchase of office equipment, lobbying activities, large group educational presentations, health screenings or health fairs without follow-up activity, or incentives or rewards for participants.
Grant requests may include indirect expenses up to 10 percent of the total request. Indirect expenses are costs the organization would incur regardless of the program, such as rent, utilities, administration and technology costs.
Organizations may apply for more than one program grant during the annual grant cycle. However, in reviewing applications, the Foundation will assess multiple proposals against other funding opportunities and consider the organization’s capacity to effectively administer multiple projects. Applicants are encouraged to review the REACH strategic plan and theory of change before preparing a grant application. In addition, applicants must follow Uniform Data System (UDS) definitions for describing and reporting on users (patients) and encounters (visits).
Availability: Program grants are awarded through an annual competitive grant cycle. Applications are available in August. Awards are determined in November of each year.
