Mental Health

Desired Outcome: All poor and underserved people in our community are reaching their full emotional and psychological potential.


In order to achieve this outcome, the Foundation has identified intermediate outcomes, or preconditions, which we believe must be in place. For each of the three intermediate outcomes stated below, the Foundation has identified a set of indicators that can be used to guide program development and measure success.

  1. Availability of quality services and providers
  2. Accessibility of services and providers to the target population
  3. Issues addressed before critical need is reached (early intervention)

The following elements are essential to eliminating barriers to health care for poor, minority and medically underserved populations:

  • A culturally competent workforce and organizational commitment to diversity and inclusion
  • Integration of oral health, mental health and primary care services
  • Public and private policies that enable fair and equitable access to comprehensive services

Note: Mental Health grantees will be expected to report service utilization data using the definitions of encounters and users per the Bureau of Primary Health Care Uniform Data System (UDS).
